In order to remove farmville just follow this steps

• Log in to your Facebook Account
• Then go to Account->Privacy Settings
• Now Click on Apps and Websites under that select Edit your settings for using  games,apps, and websites.

• Here Under Apps you use click on Farmville and Click on Remove Apps option
